REQUIRED FORMS FOR  SCHOOL YEAR 2011-2012

Required forms to be returned to the Seton main office as soon as possible and no later than Friday - August 26, 2011.  Please print out, complete, sign, and return the following EIGHT FORMS:

  1. Educational and Conduct Contract 2011-12 (acknowledging that you have read, understand, and agree to abide by the contents of the 2011-12 Seton Catholic School Family Handbook)
  2. Legal Custody Form 2011-12 (pdf)
  3. Photo Release Form 2011-12 (pdf) 
  4. Family Directory Permission Form 2011-12 (pdf)
  5. Student Acceptable Use of Technologies Policy 2011-12 (pdf) 
  6. Extended Day Program Registration Form 2011-12 (pdf) (please complete this form if you intend to use this service at any time during the year - on a regular, occasional, or emergency basis)
  7. Request for Administration of Medication by School Personnel Form 2011-12 (pdf)  (optional - form to be completed if any medications are to be administered.)
  8. Volunteer Bus Duty Form for 1st Week of School 2011-12 (pdf)  (optional - form to be completed by those willing to volunteer for this first week of school duty in Hudson)

In addition to the above forms each family will receive an envelope (to be picked up at the Parent Orientation Meeting on August 22, 2011) with four completed forms that will require verification and updating as needed.  All website and 'picked up' forms can be returned in the family envelope to the main office on or before August 26, 2011.  Thank you!
