REQUIRED FORMS 2017-2018

Required forms for the 2017-18 academic year are to be completed and returned to the Seton main office after August 7, 2017 and no later than Friday, August 25, 2017.  Please print out, complete, sign, and return the following required SIX FORMS:

  1. Educational and Conduct Contract 2017-18
    (acknowledging that you have read, understand, and agree to abide by the contents of the 2017-18 Seton Catholic School Family Handbook - one form per family)
  2. Emergency Medical Form (complete and sign a separate form for each child)
  3. Family Information and Dismissal Form (complete and sign one form per family)
  4. Legal Custody Form 2017-18 (complete and sign a form if there has been a change since 2016-17 school year or if you are a newly enrolled family entering 2017-18)
  5. Photo Release Form 2017-18 (complete and sign one form per family)
  6. Student Acceptable Use of Technologies Policy 2017-18 (complete and sign a separate form for each child)

OPTIONAL FORMS 2017-18

Please review the following documents and include with required forms if applicable to your family.

  1. Crusader Care Extended Day Program Registration Form 2017-18
    Please complete this form if you intend to use this service at any time during the year - on a regular, occasional, or emergency basis.
  2. Letter to Parents stating Medication Policy
  3. Parent & Prescriber Request for Administration of Medication Form  Optional - form to be completed if any medications are to be administered.
  4. Allergies Action Plan ~ Optional form to be completed by parents.
  5. Allergy Action Plan Authorization for Student to Carry Epinephrine Autoinjector
  6. Asthma Action Plan ~ Optional form to be completed by parents.

Family Directory Permission - a form will be sent to each family requesting information and permission to be included in the Seton Family Directory 2017-18.

NOTE:  All forms may be dropped off at the Seton main office after August 7, 2017 and during Welcome Back to Seton Events.  Mailed forms should be sent to the following address:  Seton Catholic School, 6923 Stow Road, Hudson, Ohio  44236. 

STUDENT SUPPLY LISTS 2017-2018 

  • Student Supply List 2017-18 Kindergarten

    Kindergarten parents pay a fee of $75 in lieu of purchasing school supplies. This fee will include all school supplies (pencils, crayons, markers, scissors, etc.); paper, plastic, & cleaning products; as well as craft supplies for the year.

    Students will need 1 backpack (no wheels) that will fit in the child’s locker, a water bottle, and a BATH towel for rest time.

  • Student Supply List 2017-18 First Grade

    First Grade parents pay a fee of $50 in lieu of purchasing school supplies. This fee will include all school supplies (pencils, crayons, markers, scissors, etc.); paper, plastic, & cleaning products; as well as craft supplies for the year. 

    Students will need 1 backpack (no wheels) that will fit in the child’s locker.
